Community Grants - Vocational Programs

Region 7E Mental Health Initiative Funds - Serving people with Severe Mental Illness and Severe and Persistent Mental Illness (SMI/SPMI) who are residents of Pine County or the financial responsibility of the counties within Region 7E (Pine, Isanti, Kanabec, Mille Lacs, and Chisago) who are unemployed or underemployed. These funds can be accessed for work adjustment training services. 

 

CEA (Coordinated Employability Project) - Serving people with SMI/SPMI who are residents of Pine County or the financial responsibility of one the counties within Region 7E who are unemployed or underemployed.  The focus of this funding is for job placement and follow-up services

 

DRS Referrals - DRS funds are time limited for referrals made to PHASE for individual Community Based Vocational Assessments, Performance Based Agreements (Job Placement), Work Adjustment Training, and follow up services.  PHASE is CARF accredited in Community Employment Services, including Job Development, Job Supports, and Job Site Training

 

Student Transition Evaluations - Students are referred by their school district or DRS for vocational experience as directed in the individuals IEP.  Students typically spend 4-6 weeks in a variety of community based job placements for which they are paid.  During the evaluation period the students are assessed in real work situations and at the end of the evaluation period, the individual and IEP team is presented with the evaluation report.
