PHASE offers a variety of services and employment opportunities to those facing developmental and physical challenges in their lives. Through the following Program areas, we can help people with disabilities obtain and retain employment, gain valuable training and knowledge, and experience a sense of community and teamwork. PHASE provides our consumers an innovative to employment opportunities. PHASE is nearly 100% funded through a mixture of Federal, State and County funds, under the MN State Medical Assistance Waiver Program. Under MN-MA, State funds are equally matched by Federal Funds, thereby introducing a revenue stream into the State and its Counties. |
